Payment Options

Whether you’re planning for short-term recovery or long-term care, we can work directly with you, or your insurance provider to help create a personalized care plan that fits your needs and budget.

Hospital-to-Home Program

Short-term, in-home support to ensure a safe transition from hospital or rehab to home, promoting recovery and reducing readmission risk. Caregivers provide compassionate assistance under RN guidance, helping clients regain independence safely.

Services Include:

  • RN-guided post-discharge assessment and personalized care plan

  • Daily support with activities of daily living (ADLs), including bathing, dressing, and mobility

  • Medication reminders and monitoring (non-administration)

  • Communication and coordination with physicians, therapists, and case managers

  • Education and guidance for family caregivers

  • Emotional support and companionship during recovery

RN / Licensed Professional Oversight:

  • RN visits (typically 1–2 per program) to review care plans and provide guidance to caregivers

  • Ensures clinical oversight while caregivers provide hands-on non-medical support

Duration:

  • Typically 1–2 weeks

  • Families can continue with ongoing in-home care services if additional support is needed
